Appointments & Consultations

To schedule an appointment, please contact our Scheduling Department at 214-265-2118.
Our staff will be pleased to assist you I any way possible and all efforts will be made to accommodate your schedule.

After you have made an appointment, we will call you two days in advance as a courtesy to remind you of your upcoming appointment.

Please make sure that you have the following items with you when you arrive for your appointment:

  • Insurance card(s)
  • Primary Care Physician referral (if applicable)
  • A list of current prescriptions and/or over-the-counter medications you are taking, including dose and frequency.
  • Previous blood test results and any referring physician dictation which would be applicable to this office visit
  • Co-payment (due at the time of your visit)

Prescriptions

Our policy for prescription refills is that they are made only during an office visit. We do not refill prescriptions via telephone, fax or requests from your pharmacy. This is the best way for us to ensure that all necessary quality control measures are being met and that your health is closely monitored by our physicians and staff.


Cancellations

If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient.

Emergencies

Our office provides on-call service for patient emergencies, 24 hours-a-day, 7 days-a-week by calling 214-363-5535 and our on-call physician will be contacted. In the event of an emergency, please go to your nearest hospital emergency room and ask that our practice be contacted.

Financial Policy

Our practice is a provider with most major insurance plans. We provide insurance billing as a courtesy for our patients. Please be aware that any costs not covered by insurance are your responsibility.

For services that are not covered by insurance, we request payment at the time of your appointment. Co-payments and co-insurances are due at the time services are rendered. Forms of payment accepted by the office are check, cash, insurance assignment and the following credit cards:

If you have any questions regarding which insurance plans we accept or any billing concerns prior to your appointment, please contact our business office at 214-363-5535 option 7. Questions regarding your coverage and benefits should be directed to your employer or insurance company.

Medical Records

To obtain copies of your medical records, you may contact our Medical Records Department at 214-363-5535 option 5. EAD requires you to sign a Medical Records Release Form prior to sending your records out. If you are requesting copies of records to be sent directly to another physician, there is no charge. However, there is a nominal charge for copies of medical records in all other circumstances for which payment is required in advance. We require 10 working days to complete all medical records requests.
